Pre Assessment Information

Before the Assessment:

Please complete:

• ✅ Client information & Consent form

• ✅ Pre-Assessment Survey (PAS)

During the Assessment:

• First Session:

  • For Children/Mid Teens (up to 14yo): In Person ONLY. With the young person ONLY.

  • For Mid/Late teens (15-17yo): Telehealth OR In Person. With the young person AND an accompanying person.

  • For Adults (18yo+): Telehealth OR In Person. With the ADULT CLIENT (an accompanying person is helpful but not needed)

• Second Session:

  • For Children/Mid Teens (up to 14yo): Telehealth OR In Person. With the parent/caregiver ONLY

  • For Mid/Late teens (15-17yo): Telehealth OR In Person. With the young person (if willing/interested) AND an accompanying person.

  • For Adults (18yo+): Telehealth OR In Person. With the ADULT CLIENT (an accompanying person is helpful but not needed)

• A Client information & Consent form and Pre-Assessment Survey (PAS) should be completed before the assessment session.

The Assessment:

• Two sessions required. The first child session must be completed in person, however all other sessions can be complete in person or via Telehealth (Zoom)

• For children and younger teens to ADOS-2 and ADI-R will be used be your assessor. For older teens or adults the MIGDAS-2 will be used. The assessor may vary their assessment choice if they feel this is appropriate. Prior information from the pre-assessment survey will help streamline this process and may be clarified in session.

• Behavioural observations during the session are helpful (most relevant for young persons)

• Additional surveys will be sent following the session if further information is required or if further clarification is needed.
